OVERVIEW:

Supervises the nursing staff and the day-to-day nursing activities of the facility under the leadership of the Director of Nursing,

RESPONSIBILITIES:

• Coordinate and provide total nursing care for patients
• Assess resident condition and nursing needs, sets goals, prescribe appropriate actions to meet goals and needs of the residents
• Assign care to team members in accordance with the resident’s needs and the personnel’s capabilities and qualifications
• Assess and evaluate patient’s response to care plan and prioritize and implement appropriate changes to plan
• Collaborate, assess, and coordinate discharge planning needs with other health care team members
• Oversees clinical and nonclinical acute hospital operations and continuum of care

REQUIREMENTS:

• Valid Registered Nurse License in the State of New York
• CPR/BLS certified
• 3+ years nursing experience in a nursing home or other healthcare setting
  

The social worker will work with residents in the nursing home by identifying their psychosocial, mental and emotional needs along with providing, developing, and/or aiding in the access of services to meet those needs.

RESPONSIBILITIES:

• Review facility policies and procedures as part of the facility’s interdisciplinary team to assure compliance with state and federal regulations
• Participate in reviewing and setting policies concerning resident care and quality of life
• Participate in developing facility social work policies
• Understand and meet all government requirements for social service documentation
• Document progress in meeting the psychosocial needs of residents
• Work with the interdisciplinary team and administration to promote and protect resident rights and the psychological well-being of each resident.
• Prevent and address resident abuse as mandated by law and professional licensure
• Participate in the development of a written, interdisciplinary plan of care for each resident that identifies the psychosocial needs/issues of the resident
• Ensure or provide therapeutic interventions to assist residents in coping with their transition and adjustment to a long-term care facility, including their social, emotional, and psychological needs
• Ensure or provide support and education to residents/family members/significant others to assist in their understanding of placement and facility issues in addition to referring them to the appropriate social service agencies when the facility does not provide the needed services.
• Provide groups for residents/family members/significant others as appropriate to their needs
• Coordinate the resident discharge planning process and make referrals for appropriate home care services prior to the resident’s return to the community
• Educate residents and families/significant others regarding their rights and responsibilities, effective problem solving and the extent of community, health and social services that are available to them, including those necessary for effective discharge planning.
• Review grievance log with Administrator on a monthly basis so he may sign-off on resolved grievances while investigating any open grievances

QUALIFICATIONS:

• LSW, LCSW, or LICSW in the state employed
• Prior work long-term care setting is preferred
• Must have a working understanding of social systems along with the ability to implement pertinent state and federal regulations.
  

The Nurse Educator provides educational experiences that promote competent nursing practice and quality resident care.

RESPONSIBILITIES:

• Provides educational leadership to patients and care providers to enhance specialized patient care within established healthcare setting.
• Assists patients and caregivers with educational needs, problem resolution, and health management across the continuum of care.
• Collaborates with medical providers, patient care staff, and unit management in the planning, implementation, and delivery of educational curricula.
• Initiates skills development programs within the parameters of established care models and current standards of professional practice; monitors trends and implements educational strategies to ensure compliance with quality standards and parameters.
• Provides direct specialized nursing services to an assigned group of patients, providing specialized patient care within nursing protocols and assisting in the performance of all related nursing duties.
• Develops patient care plans, incorporating evidenced-based research and national standards; consults with and serves as a resource to ensure quality patient care.
• Educates technical and patient care staff in the use of new equipment, supplies, and instruments; coordinates in-service training and workshops for appropriate staff.
• Collaborates with nursing and administrative leadership on educational content, sessions, and proposed new implementation processes.
• Maintains lesson plans and attendance sheets for validation and reference.

QUALIFICATIONS:

• Bachelor of Science in Nursing (BSN) or Master of Science in Nursing (MSN)
• Valid license as a Registered Nurse in the state of New York
• Thorough knowledge and understanding of protocol, procedures, and standards within area of nursing expertise.
• Ability to provide operational guidance and leadership to technical staff in area of specialty.
• Ability to assess educational needs and design and develop responsible curricula.
• 2+ years of experience working as a registered nurse in a clinical setting
• Previous experience in a teaching setting preferred

SUMMARY

Certified Nursing Assistant (CNA) is responsible for providing direct resident care including nursing/clinical care, personal, nutritional, and restorative care.

DUTIES

• Clean and bathe patients or residents
• Help patients use the restroom
• Maintain patients' hygiene and change bedding
• Turn, reposition, and move patients between wheelchairs, seats, and beds
• Serve meals and help patients eat their food
• Measure vital signs like blood pressure and temperature
• Listen to patient’s health concerns and report details to a RN or Doctor

QUALIFICATIONS

• Valid CNA license in the state of New York
• Ability to work weekends, evenings and flexible hours
• Previous Long-Term Care experience is preferred

OVERVIEW:

Under the leadership of the Registered Nurse, the LPN will be providing coordinated care to residents in a skilled nursing facility.

RESPONSIBILITIES:

• Provide direct care responsibilities to patients

• Administer medications and treatments timely and according to policy
• Receive, transcribe, and carry out physician orders
• Follow established procedure for charting

JOB DUTIES

• Prepare food items such as fruits, vegetables and meats by cutting, dicing and slicing them.
• Assist in preparing dishes for patients following instructions for each patient.
• Put together meals for patients keeping in mind allowed nutritional values.
• Follow instructions for meal preparations for patients with food allergies and make sure to omit any ingredient that may cause gastronomical trouble.
• Prepare food trays with items such as plates, flatware, napkins, and condiments.
• Place trays in pushcarts and physically push them through facility to assigned floors.
• Serve trays to patients by placing them on tables and assisting them in partaking of their food.
• Collect food trays and dirty dishes and cart them back to the kitchen for washing.
• Assist in washing and drying dishware, glassware, and flatware.
• Coordinate with laundry services to ensure that clean napkins and tray liners are available constantly.
• Handle kitchen garbage disposal activities by placing trash in appropriate receptacles.

QUALIFICATIONS:

• Minimum of one (1) years of foodservice related experience.
• Familiar with clinical diets.
• Culinary and/or food service experience in a health care facility an asset.
• Able to read and interpret common recipes.
• Willing and able to work well within a team environment.

JOB DUTIES

• One (1) to three (3) years of direct food service operational management experience, preferably in an acute or long-term care setting
• Knowledge of P&L accountability and contract-managed service experience is desirable
• Must be willing to participate in patient/resident satisfaction programs/activities
• ServSafe certified highly desirable.
